POLICY MEMORANDUM

SUBJECT: Civil Rights Nondiscrimination

AUTHORITY: Title VI of the Civil Rights Act, as amended; Section 504 of the Rehabilitation Act, as amended; and Title IX of the Education Amendments of 1972, 45 C.F.R. § 92.8, as well as other related laws.

EFFECTIVE: Immediately

Promoting and ensuring compliance with applicable federal civil rights laws concerning nondiscrimination based on race, color, national origin, age, disability, or sex is one of the highest priorities for Baywind Village Skilled Nursing & Rehab (“Baywind Village”). As such, Baywind Vilalge is committed to providing health care services that do not exclude people or treat them differently because of race, color, national origin, age, disability or sex.

Baywind Village will continue to incorporate civil rights concerns into the culture of our company. Employees will continue to be trained on our commitment to nondiscrimination and are expected to always adhere to this policy memorandum. Baywind Village will investigate any reported violation of civil rights laws within the company and will cooperate fully with all applicable laws regarding such reports. We will allocate financial resources to the extent feasible to ensure equal access and prevent discrimination.

We will continue to vigorously assure that all residents of Baywind Village have equal access to and receive the best healthcare possible regardless of race, color, national origin, age, sex, or disability.
